Insurance: For Virginia: I am In Network with CareFirst, United Healthcare and Anthem Blue Cross Blue Shield.  In Florida, I am in network with United Healthcare, Oscar and Florida Blue. Pay only your copay, deductible or coinsurance when I am in network with your insurance.    

For Out of Network insurance, I can bill Out of Network electronically for you or I can provide you with an invoice to submit to your insurance.  They should reimburse you directly if they approve the claim.  The fee up front will be the full rate listed above. 

I do offer a sliding scale.
